Summit New Hampshire, LLC was founded in 2009 with the aim of supporting individuals facing challenges related to Intellectual Disabilities, Brain Injury, Autism Spectrum Disorders, and other associated diagnoses. Our mission is to create tailored programs for every individual, providing them with the tools and opportunities needed to lead purposeful lives within community-based settings, while also addressing and reducing problematic behaviors.
Over the past 15 years, Summit has established a strong track record of developing effective individualized service arrangements, particularly in Enhanced Family Care (EFC) homes and other residential settings such as 24/7 staffed EFC homes (utilizing a hybrid model). We are also collaborating with Gateways Community Services (Region06) to develop an Intensive Treatment Services (ITS) facility in New Hampshire. This will support individuals with a history of high-risk behaviors currently receiving out- of-state services. Additionally, Summit partners with other vendor agencies to develop residential programs. At times of crisis, Summit assists in emergency residential placements to individuals in need. Summit also provides CSS (Community Support Services) to individuals who can live more independently.
Many of the individuals whom Summit serves exhibit a diverse range of needs, challenges, and high-risk behaviors. This includes dangerous and purposeful aggression, problematic sexual behavior, elopement, Prader-Willi Syndrome (PWS), and/or a history of involvement in the criminal justice system. In developing our individualized and customized services, Summit utilizes clinical resources and expert consultants for behavior support, plan development, data tracking, observation, individual-and behavior specific program and treatment. Our staff and providers undergo extensive training to ensure they are equipped to meet the unique needs of the individuals we serve.
